BIRTH AND DEATH
CERTIFICATES

   Printed certified copies of birth certificates are available at the Health Department for those who were born in Missouri. Birth Certificates are $15 each  and Death Certificates are $14 for the first copy and $11 for each additional copy.  Photo I.D. is required for the person requesting the certificate Persons whom may request a certificate are: Self, Mother, Father, Wife, Husband, Daughter, Son, Brother, Sister, Grandmother, Grandfather, Grandchildren, Aunt, Uncle or Nephew.   Print the application or fill out at our office.
